Schedule an Appointment

You may schedule an appointment with your primary care physician by calling the office during normal business hours. Click here to find a physician near you.

When visiting your doctor’s office, bring your health insurance card and co-payment, if applicable. If you are new to our group, please arrive approximately 30 minutes before your scheduled appointment time to complete paperwork and to be registered into our electronic medical record system. We encourage you to make an appointment as soon as possible to connect with your physician, coordinate any ongoing care, and discuss your own personal healthcare needs.

After Hours Care

We know you don’t always get sick between the hours of 8-5. Before heading to the ER, think about whether your situation poses an immediate, serious threat. If not, our After Hours Care or Nurse Advice Line could be a better fit. For more information about these services, please click our urgent care services. If you are already a patient with Providence Medical Group, you can schedule connect with your provider through our online patient portal.
