Skilled Nursing Program at St. Jude Heritage Medical Group
A multidisciplinary team is actively involved in each patient’s care.
This team is led by a St. Jude Heritage Medical Group physician who is
board certified in geriatric medicine and brings tremendous expertise
to helping the elderly and severely ill improve their health and well-being.
A nurse practitioner, social worker and nurse care manager are also involved,
helping to meet the specific needs of each patient: from arranging specialty
consultations, to home assessments and other support designed to create
a smooth transition after discharge.
Weekly care review meetings between the St. Jude Heritage medical team
and nursing facility staff help track progress and target individual needs
of patients. Particular emphasis is placed on preventing conditions that
commonly affect seniors’ health and quality of life.
St. Jude Heritage’s sophisticated electronic medical record (EMR)
ensures we provide efficient, high quality care every step of the way.
With the EMR we can quickly review test results, and verify past examinations
and procedures. It also allows us to offer highly coordinated care—during
hospitalization and after discharge—between the program’s
multidisciplinary team and the patient’s personal physician.
